What can I expect at intake as a new patient?
Starting treatment can feel overwhelming, but our intake process is designed to be simple, supportive and respectful.
When you arrive, we’ll walk you through what will happen that day and answer any questions you have. We’ll talk with you privately about your health, substance use and what you’re hoping to get from treatment. You may have a brief medical check and we’ll work with you to decide what services are the best fit for you.
Before you leave, you’ll know what your next steps are, when you’ll return and who to contact if you need support. Our goal is to help you feel comfortable, informed and supported from the very first visit.
The Intake Process:
What to bring
A photo ID (driver’s license, passport, or state-issued card), your insurance card, if available and a form of payment for co-pays or self-pay.
Medical assessment
You’ll meet with a licensed medical provider to complete a comprehensive medical exam.
Behavioral health review
Your counselor will talk with you about your substance use, your life and your treatment goals.
Care planning
Together with our providers and counselors, you’ll create a personalized plan that includes FDA-approved medications, counseling and support services.
Important paperwork
You will complete paperwork to provide consent for care and sign other required forms.
