    Offered both in-clinic (OTP) and by prescription (OBOT), buprenorphine (including Suboxone®) provides flexibility for patients. It controls cravings and withdrawal symptoms while supporting long-term stability.

  • Extended-Release Buprenorphine (Sublocade® and/or Brixadi®)

    For those who prefer a weekly or once-monthly option, the extended-release buprenorphine injection (like Sublocade® and Brixadi®) provides relief from withdrawal symptoms and cravings with just one dose.

  • Naltrexone Treatment (Vivitrol®)

    Used as part of our outpatient recovery programs (OTP and OBOT), naltrexone (including Vivitrol®) is a medication that blocks the effects of opioids and reduces cravings.

Just started 5 days ago. 1st day took about 4 hours getting paperwork, seeing a councilor, labs, etc. Got medicine after completing everything. Staff is very polite, professional, carrying. Besides the cost, it's wearing my old truck out. Im surprised I haven't broken down by now. So im going to look around and see what else is available. I hate giving this up. Im a veteran and I got the VA to approve community care for this. I hate giving that up. Any veterans needing treatment, I highly recommend this place. Especially if you live in or close to Hazard ky. Anyway that's it. I wish all the best, and these folks really want to help i believe. Update.... Just wrote a kinda negative review and it won't post up. Whats up with that ? Me and my new comer girlfriend was 18 minutes late today. Day after Thanksgiving . Wouldn't let us get our medicine. Girlfriend is very very sick. I can't Allow that. Probably won't be back. I stopped coming last time because my truck tore up, no ride. Its just to much. Drive every day from different county, 20 $ in gas and we can't dose because we are 18 minutes late. I don't know what made me think i could do it this time. Foolish I am. Have to get my girl feeling better so she will eat something. Good luck to anyone who can do the deal and be ok about it. I just couldn't. Really pisses me off about my girlfriend not dosing. I talked her into trying this place. Right now she is calling me a piece of shit lier. I hate doing what im about to do. But she is deathly sick and I just can't take it. What is the first day of treatment like?

Your first day may take 2-3 hours and includes:

  • Intake assessment: We’ll review your health history, substance use and recovery goals to understand your needs.
  • Medical evaluation: A BHG provider will conduct a physical exam, review your medical history and discuss treatment options. If appropriate, medication treatment can begin the same day.
  • Personalized treatment plan: Before you leave, our team will walk you through your treatment plan, including counseling and next steps for ongoing care.

Do I need to come in every day for medication?

No. Office-based treatment does not involve daily dosing like a methadone program. Patients will meet with a provider on a scheduled basis for medication management, counseling and check-ins.

How is office-based opioid treatment different from an opioid treatment program (OTP)?

Office-based opioid treatment (OBOT) uses buprenorphine/Suboxone® and follows a more traditional doctor’s appointment model. Unlike an OTP, OBOT does not require daily visits for medication dosing. Instead, patients meet regularly with a provider, receive prescriptions and continue treatment through scheduled follow-ups and counseling.
