HIPAA Substitute Notice

It was recently discovered that individuals currently or previously enrolled in services at the Behavioral Health Group Denver Treatment Center may have been involved in an advertent email exchange where Protected Health Information was shared outside of an appropriate release of information.

The information that was shared included the following patient-specific information:

  • Full name

  • Clinic identification number

  • Phone number

  • Email address

  • Date of birth

  • Fact the individual was a client at Behavioral Health Group (BHG)

  • Employer's name (in some cases)

The information was sent to two different State organizations who are business associates of Behavioral Health Group Denver, and there is no indication that the information has been improperly used by an unauthorized individual.

The issue has been fully investigated, and Behavioral Health Group commits to protect patients from any harm. If at any point you discover that your information has been used inappropriately, please notify us and we will work with you to make sure the proper authorities are involved as warranted by the situation. In the meantime, please be vigilant and check your credit files to protect yourself from the possibility of identity theft. Even though your address and Social Security Number were not compromised, you may investigate your credit status by contacting one or more of the three major credit reporting agencies listed below:

Behavioral Health Group has undertaken appropriate training and re-education measures for all staff involved in the occurrence, and continuing monitoring is underway to try to prevent a future similar occurrence.
